Any health care facility, as defined in § 34-12-1.1, or any physician or other health practitioner providing treatment to any person who has been injured in an agricultural accident or who has suffered an illness or poisoning related to substances or practices used for agricultural purposes shall report the accident, illness, or poisoning to the Department of Health.
