(1) Form DR-528, Notice of Ad Valorem Taxes and Non-Ad Valorem Assessments (example only), incorporated by reference in Fl. Admin. Code R. 12D-16.002 must comply with Florida Statutes § 197.3635
(2) The tax collector may include an additional statement with the notice of ad valorem taxes and non-ad valorem assessments offering an explanation of any item on the notice.
