The Alzheimer’s Community Assistance Program shall undertake the following services:

1. To establish and operate self-help support groups for family members of persons with Alzheimer’s disease residing at home.

2. To provide training programs and support services for volunteers, family members, and/or home health care workers enabling them to deliver proper care for persons with Alzheimer’s who reside at home and in the community.

3. To assist in the development of day care programs and other suitable respite services which will provide congregate care for persons with Alzheimer’s.

4. To recruit, train and supervise volunteers who will provide in and out-of-home respite care services to support family caregivers.

5. To work with the department to distribute educational materials regarding community resources and care options, including managed care as it becomes available for persons with Alzheimer’s disease.

6. To undertake outreach to families with a member suffering from Alzheimer’s to participate in counseling, education and support.

7. To work with the department of health to explore delivery mechanisms

for

family

intervention

services

to

delay institutionalization.
