- Affidavit: A written statement of facts confirmed by the oath of the party making it, before a notary or officer having authority to administer oaths.
Sec. 6. The election division shall prescribe the form of the affidavit required by this chapter that must permit the person to execute a request for transfer of the person’s registration.
