  • access practice agreement: means an agreement that:

  • access practice dentist: means a licensed dentist who is licensed under IC 25-14 and has entered into an access practice agreement under this chapter with a dental hygienist. See Indiana Code 25-13-3-3
  • Board: means the state board of dentistry established by IC 25-14-1. See Indiana Code 25-13-1-2
  • Dental hygienist: means one who is especially educated and trained in the science and art of maintaining the dental health of the individual or community through prophylactic or preventive measures applied to the teeth and adjacent structures. See Indiana Code 25-13-1-2
  • Direct supervision: means that a licensed dentist is physically present in the facility when patient care is provided by the dental hygienist. See Indiana Code 25-13-1-2
  • Guardian: A person legally empowered and charged with the duty of taking care of and managing the property of another person who because of age, intellect, or health, is incapable of managing his (her) own affairs.
  • in writing: include printing, lithographing, or other mode of representing words and letters. See Indiana Code 1-1-4-5
  • License: means the license to practice dental hygiene issued by the state board of dentistry to dental hygienist candidates who satisfactorily pass the board's examinations. See Indiana Code 25-13-1-2
  • Licensed dentist: refers to a dentist licensed under IC 25-14. See Indiana Code 25-13-1-2
  • minor: means a person less than eighteen (18) years of age. See Indiana Code 1-1-4-5
