All forms, child support orders, wage withholding orders, or orders amending an existing child support order, entered in any case in Circuit Court, District Court, or family court that require entry into the state case registry under KRS § 15.802(3) shall be entered on forms adopted by the Administrative Office of the Courts after consultation with the Office of the Attorney General. If the provisions of a child support order are contained in an order that is narrative in nature, the adopted forms shall be used in addition to the narrative order.
