  • Clinical certificate: means the written conclusion and statements of a physician or a licensed psychologist that an individual is a person requiring treatment, together with the information and opinions, in reasonable detail, that underlie the conclusion, on the form prescribed by the department or on a substantially similar form.   The hospital director shall ensure that an individual who is hospitalized under section 423 or 438 receives a copy of each clinical certificate executed in connection with the individual’s hospitalization. Each clinical certificate shall be delivered to the individual within 24 hours of either the clinical certificate’s completion or receipt of the clinical certificate by the hospital.
