The articles of amendment required by § 47-21-8 shall recite that they are executed pursuant to this chapter and shall state:

(1) The name of the cooperative;

  • Affidavit: A written statement of facts confirmed by the oath of the party making it, before a notary or officer having authority to administer oaths.
  • Amendment: A proposal to alter the text of a pending bill or other measure by striking out some of it, by inserting new language, or both. Before an amendment becomes part of the measure, thelegislature must agree to it.

(2) The address of its principal office; and

(3) The amendment to its articles of incorporation.

The president or vicepresident executing such articles of amendment shall make and annex thereto an affidavit stating that the provisions of § 47-21-8 in respect of the amendment set forth in such articles were duly complied with.
