Tennessee Code 63-12-123 – Issuance of duplicate license

(a) The board may issue a duplicate license to replace one that has been lost or destroyed or where a name change is requested, for a fee as set by the board.

Terms Used In Tennessee Code 63-12-123

Board: means the board of veterinary medical examiners. See Tennessee Code 63-12-103
License: means any permit, approval, registration or certificate issued by the board. See Tennessee Code 63-12-103

(b) Each duplicate license shall have the word “Duplicate” typed or printed across its face.
