(a) The committee shall keep an information file about each complaint filed with the committee. The committee’s information file shall be kept current and contain a record for each complaint of:
Complaint: A written statement by the plaintiff stating the wrongs allegedly committed by the defendant.
Person: includes a corporation, firm, company or association. See Tennessee Code 1-3-105
Record: means information that is inscribed on a tangible medium or that is stored in an electronic or other medium and is retrievable in a perceivable form. See Tennessee Code 1-3-105
written: includes printing, typewriting, engraving, lithography, and any other mode of representing words and letters. See Tennessee Code 1-3-105
(1) All persons contacted in relation to the complaint;(2) A summary of findings made at each step of the complaint process;(3) An explanation of the legal basis and reason for a complaint that is dismissed; and(4) Other relevant information.(b) If a written complaint is filed with the committee that the committee has authority to resolve, the committee at least as frequently as quarterly and until final disposition of the complaint shall notify the parties to the complaint of the status of the complaint unless the notice would jeopardize an ongoing investigation.(c) The committee shall develop a form to standardize information concerning complaints made to the committee. The committee shall prescribe information to be provided to a person when the person files a complaint with the committee.(d) The committee shall provide reasonable assistance to a person who wishes to file a complaint with the committee.
