Beginning January 1, 2011, an osteopathic physician licensed under this chapter shall, when requested by a consumer:

(1) make a list of prices charged by the osteopathic physician available for the consumer which includes the osteopathic physician’s 25 most frequently performed:

Terms Used In Utah Code 58-68-804

  • Physician: means both physicians and surgeons licensed under Section 58-67-301, Utah Medical Practice Act, and osteopathic physicians and surgeons licensed under Section 58-68-301, Utah Osteopathic Medical Practice Act. See Utah Code 58-68-102
     (1)(a) clinic procedures or clinic services;
     (1)(b) out-patient procedures; and
     (1)(c) in-patient procedures; and
(2) provide the consumer with information regarding any discount available for:

     (2)(a) services not covered by insurance; or
     (2)(b) prompt payment of billed charges.
