Prior to the discharge or referral of any person to any nursing facility, a hospital shall notify the department that such person requires nursing facility services which necessitate an assessment under N.H. Rev. Stat. § 151-E:7 or the provision of information and assistance under N.H. Rev. Stat. § 151-E:9.
