(a) Claims for benefits under the plan shall be filed in writing with the executive director on forms provided by the pension board in accordance with procedures established by the pension board or the executive director, or both. The procedure and documents to be produced by a member or beneficiary may differ depending on the type of benefit claim being made.

  • Beneficiary: A person who is entitled to receive the benefits or proceeds of a will, trust, insurance policy, retirement plan, annuity, or other contract. Source: OCC
  • Power of attorney: A written instrument which authorizes one person to act as another's agent or attorney. The power of attorney may be for a definite, specific act, or it may be general in nature. The terms of the written power of attorney may specify when it will expire. (b) The pension board may allow a properly designated power of attorney to act on behalf of a member or beneficiary so long as the act is authorized under the terms of the power of attorney documentation.
