The form is listed by rule number, which is also the form number, and with the title, subject and effective date. Copies of forms may be obtained from a local District Office or by writing to the Florida Department of Environmental Protection, Solid Waste Section, MS 4565, 2600 Blair Stone Road, Tallahassee, Florida 32399-2400.
    (1) Form 62-713.900(1): Application for Permit to Construct or Operate a Stationary Soil Treatment Facility, effective August 5, 1999.
    (2) Form 62-713.900(2): Notification of Intent to Use a General Permit to Construct or Operate a Mobile Soil Treatment Facility, effective August 5, 1999.
