(1) An Osteopathic Faculty Certificate may be issued by the Department to a faculty member of a school accredited by the American Osteopathic Association upon the request of the dean of the school if the faculty member has demonstrated to the Board that:

  • Jurisdiction: (1) The legal authority of a court to hear and decide a case. Concurrent jurisdiction exists when two courts have simultaneous responsibility for the same case. (2) The geographic area over which the court has authority to decide cases.
    (a) The faculty member is currently licensed to practice osteopathic medicine in another jurisdiction of the United States; and,
    (b) Is a graduate of a school of osteopathic medicine accredited by the American Osteopathic Association; and,
    (c) Files an application on board approved application form, DH-MQA 1193 (Revised 5/22), Application for Osteopathic Medical Faculty Certificate, which is hereby incorporated by reference, and may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-14611, from the Board of Osteopathic Medicine, 4052 Bald Cypress Way, Bin #C06, Tallahassee, Florida 32399-3256 or by web at https://floridasosteopathicmedicine.gov/, and otherwise meets the requirements contained in Florida Statutes § 459.0055; and,
    (d) Has submitted the application fee required by subsection 64B15-10.002(4), F.A.C.
    (2) An Osteopathic Faculty Certificate authorizes the holder to practice only in conjunction with his or her teaching duties at an accredited school of osteopathic medicine or in its affiliated teaching hospitals or clinics.
    (3) Faculty Certificates shall automatically expire upon termination of the holder’s relationship with the school or after a period of 24 months, whichever occurs first. Faculty Certificates are subject to cancellation or revocation by the Board for failure to comply with Chapters 456 and 459, F.S., and rule Division 64B15, F.A.C.
