(1) Each applicant for licensure shall pay a nonrefundable application fee payable to the Department of Health. An incomplete application shall expire 1 year after initial filing. Upon expiration of an incomplete application, an applicant must submit a new application and fee to be considered for licensure.
(2) The application fee for licensure by taking the national examination or endorsement is $100.
(3) When the Board certifies the applicant to sit for the examination, it is the applicant’s responsibility to complete the examination process with the national vendor. Pursuant to the Americans with Disabilities Act, any applicant requesting special accommodations shall comply with Department of Health Fl. Admin. Code R. 64B-1.005 (effective 11/5/14), which is incorporated by reference herein and may be obtained from the Board office or at http://www.flrules.org/Gateway/reference.asp?No=Ref-06100.
(4) The initial licensure fee is $75.
(5) The biennial renewal fee for an active license is $75.
(6) The biennial renewal fee for an inactive license is $50.
(7) The retired status fee is $50.
(8) The delinquency fee is $55.
(9) The fee for reactivation is $50.
(10) The change of status fee is $40.
(11) The duplicate license fee is $25.
(12) The certification of a public record fee is $25. Duplicating fees as specified in Florida Statutes § 119.07(4), shall also apply.
