(1) A licensee may apply to reactivate an inactive license on Form #DH 1276-MQA, Application for Reactivation (9/2015), incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-06536.
    (2) No inactive license can be reactivated unless the licensee pays the applicable fees and demonstrates completion of at least 24 hours of approved continuing education for each biennium of inactive status.
