(1) All communications concerning this part should be addressed to: Department of Health, Bureau of Radiation Control, 4052 Bald Cypress Way, Bin C21, Tallahassee, Florida 32399-1741.
(2) All notifications required to be made pursuant to Rules 64E-5.1506, 64E-5.1508, 64E-5.1509 and 64E-5.1511, F.A.C., shall be addressed to: Department of Health, Office of Radiation Control, Post Office Box 680069, Orlando, Florida 32868-0069; telephone (407)297-2095.
(3) Immediate notification as required by Fl. Admin. Code R. 64E-5.1511, shall be made by telephone or telegraph.
