(1) The remittance advice portion of the warrant form will be used to inform the payee of factual data related to the purpose for which the warrant is being issued or other information. Any information to be included on the remittance advice is subject to the prior approval of the Department.
    (2) Requests for approval of the information to be printed on the remittance advice should be submitted to the Director of the Division of Accounting and Auditing, Department of Financial Services, 200 East Gaines Street, Tallahassee, Florida 32399-0353, at least 21 days prior to the date when the information is proposed to be printed. Requests must contain the exact text of the information desired to be printed and the warrant type or subunit thereof proposed for use.
    (3) Criteria to be used in the evaluation of a request shall include:
    (a) Fundamental state purpose;
    (b) Accuracy of the information; and
    (c) Space availability.
