Sec. 5. The service provider shall give notice of denial or limitation of rights under section 4 of this chapter to the following:
(2) The guardian or appointed advocate of the patient.
(1) The patient.

- Guardian: A person legally empowered and charged with the duty of taking care of and managing the property of another person who because of age, intellect, or health, is incapable of managing his (her) own affairs.
