1. Application. An applicant seeking an initial or a renewed license must submit an application with the fee established by rule adopted by the director and any other materials required by the board.

Terms Used In Maine Revised Statutes Title 32 Sec. 18341
- Board: means the Board of Dental Practice established in Title 5, section 12004?A, subsection 10. See Maine Revised Statutes Title 32 Sec. 18302
- Director: means the Director of the Office of Professional and Occupational Regulation within the Department of Professional and Financial Regulation. See Maine Revised Statutes Title 32 Sec. 18302
- Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
2. Age. An applicant must be 18 years of age or older.

3. Time limit. An applicant has 90 days after being notified of the materials needed to complete the application to submit those materials to the board. Failure to complete the application within that 90-day period may result in a denial of the application.

4. Basic life support. An applicant seeking an initial or a renewed license must include evidence of basic life support certification.
