(1) A general permit may be revoked or modified if, after opportunity for a public hearing or a contested case hearing under the administrative procedures act of 1969, Act No. 306 of the Public Acts of 1969, being section 24.201 to 24.328 of the Michigan Compiled Laws, the department determines that the activities authorized by the general permit have an adverse impact on the environment or the activities would be more appropriately authorized by an individual permit.
  (2) A permit may be terminated or modified for cause, including:

  (a) A violation of a condition of the permit.
  (b) Obtaining a permit by misrepresentation or failure to fully disclose relevant facts.
  (c) A change in a condition that requires a temporary or permanent change in the activity.
