I. In cases involving serious injury or death to a child subject to restraint or seclusion in a facility, the facility shall, in addition to the provisions of N.H. Rev. Stat. § 126-U:7, notify the commissioner of the department of health and human services, the attorney general, and the state‘s federally-designated protection and advocacy agency for individuals with disabilities. Such notice shall include the notification required in N.H. Rev. Stat. § 126-U:7, II. The department of health and human services shall annually notify facilities of their responsibilities under this section and provide contact information for the persons to be notified.

Such notice shall include the written notification required in N.H. Rev. Stat. § 126-U:7, II. The department of education shall annually notify schools of their responsibilities under this section and provide contact information for the persons to be notified.
