Terms Used In New Jersey Statutes 34:15-128.2
- person: includes corporations, companies, associations, societies, firms, partnerships and joint stock companies as well as individuals, unless restricted by the context to an individual as distinguished from a corporate entity or specifically restricted to one or some of the above enumerated synonyms and, when used to designate the owner of property which may be the subject of an offense, includes this State, the United States, any other State of the United States as defined infra and any foreign country or government lawfully owning or possessing property within this State. “Disclose” means to release, transfer, open for inspection, make available for copying or otherwise divulge information to any person other than the individual who is the subject of the information.
“Division” means the Division of Workers’ Compensation.
“Medical information” means information, whether oral or recorded in any form or medium, that is created or received by a health care provider regarding an individual which is or may be used in connection with a workers’ compensation case, or that is provided to the employer or its workers’ compensation insurer or their agents in connection with the case, and relates to an individual’s past, present or future physical or mental health or condition, the provision of health care to an individual, or the past, present or future payment for the provision of health care to an individual.
“Workers’ compensation case” or “case” means any case in which an individual seeks workers’ compensation benefits, whether or not the individual files a formal claim with the division.
