(1) A person desiring to obtain a license or conditional license from the State Board of Examiners for Speech-Language Pathology and Audiology shall make application to the board.
Terms Used In Oregon Statutes 681.270
- Conditional license: means a license issued to an applicant under ORS § 681. See Oregon Statutes 681.205
- Person: includes individuals, corporations, associations, firms, partnerships, limited liability companies and joint stock companies. See Oregon Statutes 174.100
(2) The application shall be made upon a form and in the manner prescribed by the board.
