(a)  The state police shall draft a uniform statewide report form to be utilized by all the police departments in the state when a motor vehicle is reported stolen or is damaged by fire. Once drafted, the report shall be made available to the several police departments within the state. Every owner of a motor vehicle which is stolen or damaged by fire shall be required to complete the standardized report form according to the instructions contained therein upon the request of any police department within the state. Any person willfully failing to complete and submit a report form as required by this section may be fined one hundred dollars ($100).

(b)  No payment shall be made by an insurer for loss or damage to an insured’s vehicle until the insured has filed a report in conformity with the provisions of this section and the insurer has within thirty (30) days received said report and determined that there is no fraud involved.
