20A-2-108. Driver license or state identification card registration form — Transmittal of information.

As used in this section, “qualifying form” means:

Answer: The formal written statement by a defendant responding to a civil complaint and setting forth the grounds for defense.
Registration form: means a form by which an individual may register to vote under this title. See Utah Code 20A-1-102
State: when applied to the different parts of the United States, includes a state, district, or territory of the United States. See Utah Code 68-3-12.5
The lieutenant governor and the Driver License Division shall design each qualifying form to include:
(a)
the following question, which an applicant is required to answer: “Do you authorize the use of information in this form for voter registration purposes? YES____ NO____”;
