Terms Used In Vermont Statutes Title 26 Sec. 3356

  • Director: means the Director of the Office of Professional Regulation. See
  • Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
  • Occupational therapist: means a person who is licensed to practice occupational therapy under this chapter. See
  • Occupational therapy assistant: means a person who is licensed to assist in the practice of occupational therapy under the supervision of an occupational therapist. See
  • Person: shall include any natural person, corporation, municipality, the State of Vermont or any department, agency, or subdivision of the State, and any partnership, unincorporated association, or other legal entity. See

§ 3356. Application

To apply for licensure as an occupational therapist or occupational therapy assistant, a person shall apply to the Director on a form furnished by the Director. The application shall be accompanied by payment of the specified fee and evidence of eligibility as requested by the Director. (Added 1993, No. 102, § 1; amended 2001, No. 151 (Adj.
