(5) An outpatient physical therapy, occupational therapy, or speech pathology service provider.

(6) A comprehensive outpatient rehabilitation facility.

(7) A histocompatibility laboratory.

(8) An organ procurement organization.

(9) A religious nonmedical health care institution.
