Sec. 3. (a) Upon request by the state department, a person that owns or operates a temporary health care services agency shall provide to the state department a copy of all forms and contracts related to the fees, charges, or commissions expected to be collected by the temporary health care services agency from health care personnel or a health care facility.

     (b) Records provided to the state department under subsection (a) are not subject to disclosure under IC 5-14-3.
