(1) Any person desiring to renew an active status license to practice speech-language pathology or audiology, or a certificate as a speech-language assistant or audiology assistant, shall apply to the Department of Health. The application for renewal shall be made on a form, prepared and furnished by the Department.
    (2) The Department shall renew only those licensees or certificate holders who have completed the application form, remitted the appropriate renewal fee established by Fl. Admin. Code R. 64B20-3.007, and who have demonstrated that they have met the appropriate continuing education requirements established by Fl. Admin. Code R. 64B20-6.001
