Every applicant for a notarial commission shall sign the following declaration in the presence of a notary:
“Declaration of Applicant

I,

(name of applicant), solemnly
swear (or affirm) under penalty of perjury that the personal information in this application is true, complete, and correct; that I have read carefully the materials describing the duties of a notary in Guam; and that I will perform, to the best of my ability all notarial acts in accordance with law.
