- Affidavit: A written statement of facts confirmed by the oath of the party making it, before a notary or officer having authority to administer oaths.
Any caregiver who makes a false statement in the affidavit for caregiver consent required under § 302A-482, shall be subject to the penalties under part V of chapter 710.
