A victim of sexual assault or sexual abuse may give a person consent to provide information about the sexual assault or sexual abuse to a law enforcement officer, and the officer shall complete a written report unless:
         (1) the person contacting law enforcement fails to
    
provide the person’s name and contact information; or
        (2) the person contacting law enforcement fails to
    
affirm that the person has the consent of the victim of the sexual assault or sexual abuse.
