- health insurance policy: means any individual or group accident and sickness policy, contract, subscriber agreement, rider, endorsement, or any contract providing for the delivery of health care benefits, delivered or issued for delivery in Indiana after June 30, 1996, by any of the following:

Sec. 2. As used in this chapter, “health insurer” means an entity referred to in section 1 of this chapter that issues, delivers, or renews a health insurance policy.
