Each small employer carrier shall provide all of the following to a small employer upon request and upon entering into a contract with the small employer:
  (a) The extent to which premiums for a specific small employer are established or adjusted due to any permitted characteristic and rating factors of the small employer’s employees and the employees’ dependents.

  • Carrier: means a person that provides health benefits, coverage, or insurance in this state. See Michigan Laws 500.3701
  • Contract: A legal written agreement that becomes binding when signed.
  • Small employer: means any person actively engaged in business that, on at least 50% of its working days during the preceding and current calendar years, employed not fewer than 2 and not more than 50 eligible employees. See Michigan Laws 500.3701
  (b) The provisions concerning the carrier’s right to change premiums, permitted characteristics, and any rating factors under this chapter that affect changes in premiums.
  (c) The provisions relating to renewability of coverage.
