Every assessable policy delivered or issued for delivery in this state by an insurer, together with any application therefor shall have conspicuously printed near the top of the face of such policy and application thereof in boldface type of a size not less than the largest type used for any heading or caption in the policy or application, the words “Important: this is an assessable policy,” or “Important: this is an application for an assessable policy.”
