Plea agreement (definition)

An arrangement between the prosecutor, the defense attorney, and the defendant in which the defendant agrees to plead guilty in exchange for special considerations.

If a person accepts a plea but was not properly informed by the defense counsel of all it's conditions, nor did the defendant ever accept the plea based on guilt but only on the advice that counsel gave, that by accepting the plea, it was in the defendant's best interests... what can be done to rectify this? It has been years later and the defendant had no idea of the requirements of accepting this plea and avoiding trial???
