(1) Each employing agency seeking reimbursement for supplemental compensation paid to firefighters pursuant to Florida Statutes § 633.422, and this rule shall submit quarterly reports to the division on March 31, June 30, September 30, and December 31 of each year on Form DFS-K4-1065, “”Quarterly Report/Firefighters Supplemental Compensation,”” which is incorporated by reference in Fl. Admin. Code R. 69A-47.090, http://www.flrules.org/Gateway/reference.asp?No=Ref-01639.
    (2) Every employing agency must maintain written records relating to the eligibility of every firefighter whose name is submitted to the division pursuant to subsection (1), above, in separate files for a period of five years for audit purposes.
