(1) A person applying for a certificate of authority as a prepaid limited service organization shall submit the following:
(a) Form OIR-C1-1119, “”Application for Certificate of Authority Prepaid Limited Health Service Organization,”” effective 07/23, hereby incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-16241;
