state of public health emergency shall be declared by an executive order that specifies:
(a) the nature of the public health emergency;
(b) the geographic area(s) applicable to the declaration; (c) the conditions that have brought about the public
health emergency;
(d) the duration of the state of the public health emergency, if less than thirty (30) days; and
(e) the primary public health authority responding to the emergency.
