Indiana Code 24-5-18-8. Onsite reversal test procedure; warning tag; notice
(b) If the automatic garage door opening system does not pass the onsite reversal test required by subsection (a), the supplier conducting the test shall complete and conspicuously attach to the automatic garage door opening system a label or tag that contains the following information (The following language complies with this requirement, but other language may be used that conveys the equivalent information.):
Terms Used In Indiana Code 24-5-18-8
- automatic garage door opening system: means a system of devices and equipment that is not a commercial garage door operator and that when connected to a garage door automatically opens and closes the garage door. See Indiana Code 24-5-18-1
- garage: means a building or a part of a building designed or used for the storage, repair, or keeping of a motor vehicle. See Indiana Code 24-5-18-3
- in writing: include printing, lithographing, or other mode of representing words and letters. See Indiana Code 1-1-4-5
- person: means an individual, a corporation, a partnership, an association, or other legal entity. See Indiana Code 24-5-18-4
- residential building: means a structure such as a home or an apartment for one (1) or more families or persons that includes an attached or unattached garage. See Indiana Code 24-5-18-5
- supplier: means a seller, lessor, assignor, or other person who regularly engages in or solicits consumer transactions. See Indiana Code 24-5-18-6
This garage door opener was tested and does not meet the current requirements for a working safety reverse feature. This can be dangerous and may cause serious injury or death. You are advised to disconnect the opener from the door immediately and operate the door manually until the opener has been repaired or replaced with an opener that meets current safety standards relating to automatic reversal.

(c) The word “WARNING” on the label or tag described in subsection (b) shall be in the foreground and background colors that comply with the standards of the American National Standards Institute (ANSI) that are in effect at the time that the label or tag is affixed.
(d) The employer of the person who attaches a label or tag under subsection (b) shall notify the occupant of the residence within ten (10) working days in writing that the residence’s automatic garage door opening system does not comply with this chapter.
