  • Affidavit: A written statement of facts confirmed by the oath of the party making it, before a notary or officer having authority to administer oaths.

  • unsworn declaration: means a declaration in a signed record that is not given under oath but is given under penalty of perjury. See Indiana Code 34-59-1-8
