Sec. 14c.

(1) Not later than 10 days after the entry of an order of adoption pursuant to section 56 of the adoption code, being section 710.56 of the Michigan Compiled Laws, the primary adoption facilitator for that adoption shall file with the probate court a completed public information form setting forth information including costs connected with the adoption as prescribed by section 14d. The public information form shall be authenticated by verification under oath by the primary adoption facilitator, or, in the alternative, contain the following statement immediately above the date and signature of the facilitator: “I declare that this public information form has been examined by me and that its contents are true to the best of my information, knowledge, and belief.”

(2) This section does not apply to a stepparent adoption, the adoption of a child related to the petitioner within the fifth degree by blood, marriage, or adoption, or an adoption in which the consent of a court or the department is required.

(3) Except as provided in subsection (2), this section applies to adoptions in which the order of adoption under section 56 of the adoption code is entered after the effective date of this section, including adoptions pending on the effective date of this section.
