50-32-403. Common reporting form. (1) The department of justice must provide a common reporting form for the report required under 50-32-401.

Department: means the department of labor and industry provided for in Title 2, chapter 15, part 17. See Montana Code 50-32-101
Person: means an individual, corporation, government or governmental subdivision or agency, business trust, estate, trust, partnership, association, or any other legal entity. See Montana Code 50-32-101

(2)The form must contain the following information:

(a)name of the substance;

(b)quantity of the substance sold, transferred, or furnished;

(c)the date the substance was sold, transferred, or furnished;

(d)the name and address of the person buying or receiving the substance; and

(e)the name and address of the manufacturer, wholesaler, or retailer.
