§ 4137. Births; notice of recording. 1. (a) Within ten days after receiving the certificate of birth the registrar shall furnish without charge to the parents or guardian of the child or to the mother at the address designated by her for the purpose, a certified copy.

  • Guardian: A person legally empowered and charged with the duty of taking care of and managing the property of another person who because of age, intellect, or health, is incapable of managing his (her) own affairs.

(b) The registrar shall, at the same time, furnish without charge to the parents or guardian of the child or to the mother at the address designated by her the immunization schedule for children. The registrar shall include the telephone number of the local county health department on the immunization schedule.

2. A certificate of registration of birth shall be accepted by public authorities in this state for the purposes indicated in section four thousand one hundred three of this chapter in the same manner as certified copies or certified transcripts of birth certificates.

3. The local registrar shall also make a notation on his copy of the original birth certificate indicating the date of issuance of such certified copy.
