The application shall be verified and shall include the following:
(a) The full name, residence address, telephone number, and date of birth of the employee.
Terms Used In California Business and Professions Code 6980.44
- Employee: means an individual who works for an employer, is listed on the employer's payroll records, and is under the employer's direction and control. See California Business and Professions Code 6980
- Employer: means a person who employs an individual for wages or salary, lists the individual on the employer's payroll records, and withholds all legally required deductions and contributions. See California Business and Professions Code 6980
- license: means license, certificate, registration, or other means to engage in a business or profession regulated by this code or referred to in Section 1000 or 3600. See California Business and Professions Code 23.7
(b) The name, address, telephone number, and license number of the employer, and the date the employment commenced.
(c) A statement as to whether the employee has been arrested or convicted of a misdemeanor, excluding minor traffic violations.
(d) A statement as to whether the employee has been convicted of a felony.
