(a) The company shall establish methods by which consumers and service recipients are notified of the name, mailing address, and telephone number of the company for the purpose of directing a complaint to the company.
(b) The company may provide for the notice:
(1) by a supplement or endorsement to a written policy;
(2) on a sign prominently displayed in the place of business of each regional office of the company; or
(3) in a bill for services provided by the company.
