An applicant may be acknowledged as a graduate from a board approved massage therapy school pursuant to Fl. Admin. Code R. 64B7-25.001(1)(c) when proof of graduation is demonstrated by the submisson of:

Terms Used In Florida Regulations 64B7-32.002

  • Transcript: A written, word-for-word record of what was said, either in a proceeding such as a trial or during some other conversation, as in a transcript of a hearing or oral deposition.
    (1) A graduate list from a board approve massage therapy school in Florida which identifies the graduate, or
    (2) An official transcript provided by a board approved massage therapy school, or custodian of records in the event of school closure which:
    (a) Indentifies the graduate,
    (b) Identifies the board approved massage therapy school,
    (c) Identifies the campus of facility by address where classroom hours were completed,
    (d) Indicates dates of enrollment and graduation,
    (e) Indicates that the graduate has successfully completed a course of study which meets or exeeds the requirements of Fl. Admin. Code R. 64B7-32.003(1)(c) by successfully completing all classes comprising the completed course of study during enrollment.
    (f) Is signed and dated by an approved signer and specifies the name and title of the approved signer.
    (g) Is sent directly from the school or custodian of records to the Board office in an envelope sealed by the approved signer or custodian or records and marked “”official transcript.””.
