Sec. 1. When assistance is granted to a dependent child under IC 12-14-1 through IC 12-14-8, the award made must be entered on a written notice prescribed by the division that designates the following:
(2) The amount of the award.
(1) The name and residence of the recipient.

(4) Any other fact required by the division.
